| SCOPE's
Supportive Services to older adults are carried out by our
Social Services Department. All of SCOPE's social workers are
licensed by the State of Ohio and are required to receive
continuing education on an ongoing basis. License review and
renewal are conducted every two years.
SCOPE's Social Services
encompasses three primary areas:
SHORT TERM COMMUNITY OUTREACH
- The dual purpose of Community Outreach is to identify
isolated and needy clients and inform clients of SCOPE
services. SCOPE also has a contract with the Trumbull County
Office of Elderly Affairs for Nutrition Outreach. Warren
SCOPE Center is a nutrition site and lunch is served in the
Community room Monday - Friday.
Outreach can be done by
inquiry, referral or through the initiation of a SCOPE social
worker. If you are aware of an individual who may need our
help, please call our Information and Referral Office at
399-8846.
SUPPORTIVE SERVICES - Supportive services done by
SCOPE social workers consists of assessing a client's total
needs, developing an individual case plan and referring the
client to SCOPE departments or other agencies. When a SCOPE
social worker makes a home visit to potential or current Home
Delivered Meal client as a part of Nutrition Outreach,
coordination assessment can be done as an integral part of the
visit.
Individual inquiries and
referrals from interested parties and other agencies and
organizations come through SCOPE's Information & Referral
Department. If determined to be a Supportive/Social Services
case, the referral is then made to the Supportive/Social
Services Supervisor, who will make arrangements to visit the
client in their home. From that point on, the social worker
will coordinate whatever services are required by the nature
of the case, making contact with the client's caregiver's when
appropriate.
An Individual Service Plan is
written when a client has multiple handicaps and little or no
outside support.
MASS
OUTREACH - The goals of Mass Outreach are
three-fold: To inform clients of services and the process
involved in receiving them, to educate older adults and to
advocate for older adults.
Mass Outreach as the term
suggests is educational and informative outreach to large
gatherings. It can take the form of Nutrition Outreach and
Trumbull County's ten nutritional sites for those over sixty
years of age, displays at health fairs and similar events,
presentation to gathering about SCOPE or other topics.
